Very Important Medical Forms
ALL MEMBERS PLEASE READ:
First is a Medical History form which is MANDATORY for ALL members of SoH
This is a form about NIHL or Noise Induced Hearing Loss
This is general medical informational form

Season Ticket Forms

If the entire SOH reaches our goal (about $25,000 in sales, an average of about 1 basic package per student), a donor has agreed to send 50 additional SOH members. Students will be chosen for this group based on priority within the following sales tiers:
Tier 1 - $400 or more in sales
Tier 2 - $280 or more in sales
Tier 3 - $210 or more in sales
Tier 4 - $140 or more in sales
Remember, if the Spirit of Houston reaches our sales goal, we'll also receive possible donations from additional alumni groups, allowing even more students to travel to UCLA. Our goal is to take the entire Spirit of Houston, but at this time, you can secure your spot by simply selling season ticket packages and helping fill Robertson Stadium for each game during its last season.
Please do not collect cash! Only credit card or checks. These forms need to be returned to Troy Bennefield by August 22nd.

MSM Fall 2012 Auditions
1. Audition times were assigned according to the enrollment list provided by the Moores School of Music administrative staff. Changes, corrections, and additions must be emailed to Cristina Mendoza, Doctoral Wind Conducting GA, at cristina.e.mendoza@gmail.com.
2. Check the list regularly; your audition time may move slightly earlier or later.
3. Plan to arrive at least 15-20 minutes early for your audition in the event that the rooms run ahead of schedule, and be aware that the rooms may also run late.

NEW REHEARSAL SCHEDULE FOR FALL 2012
For the Fall 2012 season the weekly rehearsal scheduled has changed to 6pm-8pm Monday, Wednesday, and Friday. There will occasionally be Fridays off, at the discretion of the staff.
